Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

Our Delaware County, Pennsylvania location is seeking a dedicated Professional to fill the following Full Time Peer Support Specialist interested in joining our Homeless Outreach team in Delaware County, PA. The Peer Support Specialist is responsible for engaging and building trust with service participants, deliver peer support services, and model recovery and resiliency. This position receives supervision from the Homeless Outreach Team Leader and is responsible for:

  • Serving as a mentor to program participants to promote hope and empowerment in their recovery process.
  • Collaborating with the team to promote a team culture in which self-determination and decision-making in recovery planning are supported.
  • Assisting program participants in identifying, understanding and combating stigma and discrimination associated with mental illness and develop strategies to reduce self-stigma.
  • Increase awareness of and support program participant's access and involvement/participation in self-help programs and consumer advocacy organizations that provide additional recovery resources and supports.
  • Acts as an informal educator to other team members to facilitate understanding and empathy with each person unique and subjective experience and perceptions.
Job Qualifications:

Bachelor’s degree in Human Services/Social Sciences, Healthcare or a related field and 3 years of experience in  the field, or any combination of experience, education or training that would provide the level of knowledge, skill and ability.

Certified Addiction Counselor (CAC), preferred or on-going coursework in Drug and Alcohol Counseling to qualify for certification.

Knowledge and experience in conducting street outreach; understanding of the Coordinated Entry System (CES)and its overarching goal of achieving “functional zero” for those experiencing homelessness.

Knowledge of multiple support systems for substance abusers and familiarity with recovery models.

Very good communication skills, both oral and written.

Ability to work independently and as part of a focused team.

Possession of a valid driver's license with an acceptable driving record and the ability to drive a passenger van.

Demonstrated ability to be flexible and creative in meeting the needs of participants while maintaining professional boundaries.


Frequent talking and listening.

Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.

May require crisis intervention in emergency situations.

May require working varied shifts to fulfill the mandate of Emergency Code Outreach Project and the Coordinated Evening Outreach Initiative.
