Job Description/Essential Functions:
Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking qualified Rehabilitation Counselor interested in joining our Community Residential Rehabilitation team in East Norriton, PA.

The Rehabilitation Counselor provides direct support, counseling and supervision of all assigned residents to ensure a safe, healthy, secure and a pleasant environment. Facilitate client growth and development toward independence in self-care, daily living skills, socialization and community living. The Rehabilitation Counselor facilitates coordination and arrangement of the provision of specialized services. The successful candidate’s responsibilities include the following:

• Provide guidance and support to all participants; teach and reinforce skill development in the areas of nutrition, basic self care, routine medical care and follow-up, as developed through the joint planning sessions of the individual support plans.
• Ensure the overall safety of assigned participants by maintaining a hazard free environment and avoiding potentially dangerous situations.
• Maintain and monitor medications consistent with individuals’ self-medication plans, in compliance with State and Agency medication administration regulations.
• Participate in the resolution of crisis situations; assist with identifying key individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.
• Complete, file and maintain all required documentation, which includes daily logs, operational records and other related forms as assigned by the Program Manager.
Job Qualifications:
• A.A. degree in the Social Sciences Healthcare or a related field and at least 1 year of relevant experience in the MH/MR field; or any combination of experience, education or training that would provide the level of knowledge, skill and ability required.
• Demonstrated crisis intervention/prevention and de-escalation skills required.
• Proficient communication skills, both oral and written.
• Ability to work independently and as part of a team.
• Some knowledge of the mental health system and supports.
• Valid driver’s license with an acceptable driving record required.
• Frequent talking and listening.
• Requires the ability to lift carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with residents.
• Possess manual dexterity and fine motor skills.
• May require crisis intervention in emergency situations.
• May be required to report to work during emergencies including inclement weather.
