Job Description/Essential Functions:

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,500 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting.

Currently, we are seeking a Full Time 
Certified Nursing Assistant (CNA) with experience working with medically fragile individuals in a residential/community setting.

The essential function of this position is to:

To provide direct care and supervision to participants in the residential group home setting.

To ensure a safe, healthy, secure and recovery oriented environment, which promotes growth and development.

To facilitate participant growth and development toward independence in the areas of self care, daily living, socialization and community integration.

To participate in the development and implementation of the participants’ recovery plans.

Deliver patient care utilizing the nursing process including assessment, planning of care, implementation, resident and family education and evaluation of care. 

Instruct, counsel and provide support to assigned residents, focusing on client independence and individual needs.

Maintain an environment which encourages proactive decision making and growth for each.

Job Qualifications:

High school diploma required with current certification in nursing assistance with no findings or convictions of resident abuse, mistreatment, neglect or misappropriation of participant property.

Knowledge and ability to provide hands on assistance with activities of daily living (ADL) and instrumental activities of daily living (ADL).

Knowledge and ability to develop effective rehabilitative relationships with participants receiving supports and services.

Preferred characteristics/abilities: Experience in mental health services or comparable direct service populations.

Ability to respond to disruptive behavior, agitation and depression.

Evidence of attitude to work in an environment that requires patience, flexibility, and initiative.

Proficiency in written and verbal skills.

Possess a valid driver’s license with an acceptable driving record.

Requires crisis intervention techniques in crisis situations.

Possesses manual and dexterity and fine motor skills.

Requires the ability to lift,carry, fold, kneel, reach, stack, stoop, and bend which includes directinterventions with participants.

Ability to climb and descend stairs to a second or basement level.
