Job Description/Essential Functions:

Horizon House, a non-profit organization, has been widely recognized for its services as a provider of community-based behavioral health, intellectual disabled and homeless services in Eastern Pennsylvania and the state of Delaware. Each year, Horizon House provides a continuum of services at over 100 sites to over 4,500 adults with psychiatric or developmental disabilities, drug and alcohol addictions, and/or homelessness. A pioneer in the field of community-based rehabilitation, Horizon House is widely recognized for its innovative approach to the delivery of services in a community setting. Currently, we are seeking a Full Time Certified Nursing Assistant (CNA) with experience working with medically fragile individuals in a residential/community setting.

The essential function of this position is to:

-To provide direct care and supervision to participants in the residential group home setting.

-To ensure a safe, healthy, secure and recovery oriented environment, which promotes growth and development.

-To facilitate participant growth and development toward independence in the areas of self care, daily living, socialization and community integration.

-To participate in the development and implementation of the participants’ recovery plans.

Job Qualifications:
  • High school diploma required with current certification in nursing assistance with no findings or convictions of resident abuse, mistreatment, neglect or misappropriation of participant property.
  • Knowledge and ability to provide hands on assistance with activities of daily living (ADL) and instrumental activities of daily living (ADL).
  • Knowledge and ability to develop effective rehabilitative relationships with participants receiving supports and services.
  • Preferred characteristics/abilities: Experience in mental health services or comparable direct service    populations.
  • Ability to respond to disruptive behavior, agitation and depression.
  • Evidence of attitude to work in an environment that requires patience, flexibility, and initiative.
  • Proficiency in written and verbal skills.
  • Possess a valid driver’s license with an acceptable driving record.
