Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a full-time Clinical Specialist to monitor the clinical status of individuals in the Wellness Alliance program, and Provide counseling, clinical support, service coordination, and develop/implement self help groups.

  • Develop trainings for the purpose of providing life skills, mental health, and D&A education to consumer.
  • Monitor consumer behavioral health treatment, network with the community providers regarding behavioral health care needs and work with the Wellness Alliance team to provide services and referrals consistent with evidence based practices.
  • Works in conjunction with Wellness Alliance staff to coordinate the behavioral health care services to program clients.
  • Works with Rehabilitation Coordinator to coordinate recovery/community integration, plan development, and implementation.
  • Conduct individual, group, and family counseling.
  • Available to provide intervention, assessment and referrals of individuals in crisis.
Job Qualifications:
  • Masters level degree in Social Work, Social Sciences or related field with at least three (3) years experience in Human Services, Mental Health and Addiction; or any combination of experience, education or training that would provide the level of knowledge and abilities required.
  • Comprehensive knowledge of Psychiatric Disabilities, recovery principles and psychosocial rehabilitation practices with the ability to teach and explain these areas as needed.
  • Excellent communication skills, both verbal and written.
  • Strong interpersonal skill with the ability to interact with staff, consumers and related providers.
  • Strong organizational, analytical and research skills.
  • Strong computer skills, including proficiency in MS Word and Excel.
Scope:
  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, reach, stack, stoop and bend, which may include direct intervention with residents.
  • Must be available for local travel.
  • Must possess valid driver's license.
  • May require crisis intervention in emergency situations.
  • Requires flexibility in scheduling and ability to do 24 hour on-call coverage.
