Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a full-time Clinician-Registered Nurse  to service clients with the responsibility for Clinical/Nursing support to Outpatient Services.

Responsibilities to help participants accomplish this goal include the following:

  • Complete intake assessments and present to Medical Director in A M Intake Conference.
  • Maintain weekly contact up to one month to ensure of involvement in treatment.
  • Complete BPS histories and evaluations as scheduled. Complete medication F/re-visits in consultation with treating psychiatrist.
  • Treatment plans for medication only clients as assigned. order needed medical supplies thro administration manager.
  • Evaluate patient emergencies/urgent for treatment or referral.
  • Facilitate medication renewals.
  • Assess time of last visit with psychiatrist and facilitate visit if not recent or new clinical situation warrants urgent
Job Qualifications:
  • A Registered Nurse.
  • Experience in mental health rehabilitation.
  • A demonstrated ability to monitor and assess client progress and maintain records for client services.
  • Evidence of the ability to work independently and to exercise responsibilities initiative.
  • Ability to work independently and as part of a team.


  • Requires flexibility in scheduling.
  • Requires ability to speak clearly, hear easily.
  • Requires ability to lift, carry, fold, kneel, reach, stack, stoop, bend and climb stairs.
  • May be required to report to work during emergencies including inclement weather.
