Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. 

We are seeking a qualified Outreach Specialist interested in joining our Homeless Outreach team in Delaware County, PA. The Outreach Specialist as part of the Homeless Outreach team provides intake, and assessments for participants to determine service needs and eligibility as well as Inform participants of available and acceptable service intervention options.  Some of the responsibilities include the following:

  • Provides outreach services to homeless individuals encountered on the street.
  • Engages homeless persons to accept appropriate services, which may include referrals to shelter or other supports and or facilitation of voluntary and involuntary hospitalizations.
  • Connects participants with housing placement combined with supportive services to help people transition out of homelessness and retain long-term housing stability.
  • Ensures that the program is in compliance with applicable standards. 
  • Maintains and submit all required documentation, on a timely basis, including individual client files intake assessments, movement through the service delivery system and the number of contacts made during outreach.
  • Perform outreach services to homeless individuals by utilizing local resource networks for referrals. May provide outreach and engagement services by working split and/or evening shifts.
  • Work closely with all cooperating agencies, housing and treatment facilities and advocacy or support groups with whom placements have been secured; emphasize commitment and assistance to participants upon discharge or in the event of a relapse into homelessness or substance abuse throughout the recovery process.
  • Provides intake, and assessments for participants to determine service needs and eligibility. Inform participants of available and acceptable service intervention options.
  • Maintain a close liaison with the local community and area service providers/agencies to facilitate program cooperation and provide for constructive input and/or alternative resources to current plans.
Job Qualifications:
  • Bachelor’s degree in Human Services/Social Sciences, Healthcare or a related field and 3 years of experience in  the field, or any combination of experience, education or training that would provide the level of knowledge, skill and ability.
  • Certified Addiction Counselor (CAC), preferred or on-going coursework in Drug and Alcohol Counseling to qualify for certification.
  • Knowledge and experience in conducting street outreach; understanding of the Coordinated Entry System (CES)and its overarching goal of achieving “functional zero” for those experiencing homelessness.
  • Knowledge of multiple support systems for substance abusers and familiarity with recovery models.
  • Very good communication skills, both oral and written.
  • Ability to work independently and as part of a focused team.
  • Possession of a valid driver's license with an acceptable driving record and the ability to drive a passenger van.
  • Demonstrated ability to be flexible and creative in meeting the needs of participants while maintaining professional boundaries.

  • Frequent talking and listening.
  • Requires the ability to lift, carry, fold, kneel, reach, stack, stoop and bend which may include direct intervention with participants.
  • May require crisis intervention in emergency situations.
  • May require working varied shifts to fulfill the mandate of Emergency Code Outreach Project and the Coordinated Evening Outreach Initiative.
