Job Description/Essential Functions:

Horizon House, Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues, and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.  Currently, we are seeking a full time Health Coordinator (Registered Nurse) to work in our Developmental Disabilities Division.

In this nursing position you will have the primary responsibility for coordinate service systems and operations related to the medical care of participants, in a community setting,and to provide technical expertise and training to non-medical program staff. Some of the duties are:

 > Overseeing and/or provides the implementation of nursing, therapeutic, and consultative clinical services to assigned participants, to include  participants with complex medical and/or psychiatric disabilities.
 > Write Medical History Summaries and annual reviews. Prepare medical portion of ISP summary and oversee quarterly reviews.
 > Develop central and residential medical records systems. Instruct support staff in the use of medical records systems. Monitor medical records. 
 > Provide data entry for record keeping system.
 > Provide in-service training on medical topics, e.g., infection control; HIV/AIDS; Record Keeping; communicating with physician; confidentiality; medication administration.
 > Coordinate medical care for participants who have medically complex needs and for participants who have acute problems which become medically complicated.
 > Coordinate information flow and communication between Specialist and Primary Care physicians and between professionals and program support staff. Facilitates questions regarding medical diagnosis or treatment plans; interface with physicians and relay appropriate information to support staff.

Job Qualifications:
To be successful the applicant must have:
> A Bachelor's degree in Nursing;
> Registered Nurse with experience in community nursing and/or developmental disabilities.
> Good organizational skills, very detail oriented and good communication skills.
> Knowledge of computer software for record keeping, form development, word processing, and experience with data base design and maintenance.
> A valid driver's license with an acceptable driving record.
> Able to travel to different locations throughout the tri-state area.
