Job Description/Essential Functions:

Horizon House Inc. is a nationally recognized provider of services to adults with support needs due to developmental disabilities, behavioral health issues and substance abuse. Horizon House is Pennsylvania’s oldest psychiatric rehabilitation agency. Since 1952, Horizon House has been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities and homelessness to help stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.

We are seeking a qualified candidate interested in joining our Assertive Community Treatment (ACT) team at our Georgetown location.

As a part of the Assertive Community Treatment (ACT) team the Co-Occurring Specialist will provide rehabilitation and support functions and assist in treatment, provide substance abuse services, education, support and consultation to families; and provide crisis intervention under clinical supervision.

Some of the responsibilities would include the following:

  • Provide service coordination (case management) for assigned group of participants including coordinating and monitoring the activities of the individual’s treatment team; assume primary responsibility for developing, writing, implementing, evaluating and revising the overall treatment goals.
  • Provide on-call crisis intervention covering night time hours.
  • Provides symptom education to enable participants to identify their mental illness symptoms.
  • Coordinate with state vocational rehabilitation and other employment services.
Job Qualifications:
  • Bachelors degree (Master degree preferred) in a human service field such as nursing, psychology, counseling, rehabilitation, social work with one of the following: Certified Alcohol and Drug Counselor (CADC) in the state of Delaware or Certified Co-Occurring Disorders Professional in the state of Delaware.

  • Knowledge of mental health and substance abuse systems and supports.
  • Ability to work independently.
  • Excellent communication skills bot oral and written.
  • Possess a valid drivers license with an acceptable driving record.


  • Frequent talking and listening.
  • Requires the ability to lift, carry, hold, kneel, reach, stack, stoop, and bend which may include direct intervention with participants.
  • Local travel required, including transporting program participants.
  • Requires carrying a cell phone and being available to respond to emergency situations.
  • May be required to report to work during emergencies, including inclement weather.
  • May require crisis intervention in emergency situations.
